Building Bridges: Promoting The Care Continuum

This presentation will describe a Quality Improvement Initiative implemented at AdCare Hospital to facilitate patients' transition to outpatient treatment from inpatient treatment. The need for this PI initiative was identified by various parts of the AdCare system, including Outpatient Directors (noting high no-show rates for patients referred), and the Admissions Department (reporting readmission requests from patients who had not followed their discharge plan). The Team identified a series of "Points of Contact" throughout the patients' treatment process, which educated/reinforced the need for continuing care; and developed a "Day of Discharge Group" to discuss the aftercare plan, identify barriers to implementation, and enhance motivation for participation. A "Treatment Continuity Questionnaire" was administered pre-post implementation of the PI Initiative to assess changes in patients' perception of the need for continuing treatment, and influences on such. Data on show rates and treatment plan acceptance within the AdCare system will be presented as well as other refinements made within the system to enhance the outcome.

Jack J. Maroney, MS, LADC1, Community Services Representative for Adcare Hospital

Mr. Maroney is a Licensed Alcohol and Drug Counselor and holds a master's degree in Healthcare Administration. Jack has served as a Data Coordinator and a Change Leader for NIATx 200 initiatives; a national pioneering collaborative aimed at improving treatment access and outcomes. He is President of the Massachusetts Sober Housing Corporation Board of Directors, a member of the leadership committee for the City of Worcester's Department of Public Health's Opioid Overdose Prevention Coalition and co-founder of the Latin American Health Alliance; an organization focused on eliminating health disparities in the Central Massachusetts community.

 

 
Jane M. Jolly, Director of Outpatient Operations for AdCare Hospital

Ms. Jolly has 20 years of experience in hospital administration. Her experience encompasses process improvement in the revenue cycle, patient access and EMR Jane has served as a Change Leader for Niatx 200 initiative with in the organization. She is a member of the Medical Group Management Association.
